1 leggy tube of choice, mine is from art by Barbara Jensen and is used with a purchased license.
Fonts of choice, mine are Incarnation and Altamonte NF for the letters in the stars

You should have some working knowledge of PSP.

Open a blank canvas, 500x400px and floodfill with white.
Activate your preset shapes tool and set to rectangle, set your background colour to black, foreground to white, width 3, create as vector. Draw out a square on your canvas. Convert to raster layer and add a dropshadow.
V minus4, H minus4, Opacity 41, blur 6.93. Add another dropshadow, change H and V to 4. Return to your shape tool, fill nil, stroke set to white, width 1. Draw some rectangles of differing width onto your black square. Make sure the top and bottom always blends into the white frame. Convert to rasterlayer. Hide the white layer and merge visible.

Paste on the leggy tube and resize if necessary, for placement see my screenshot ;o) Add a dropshadow of V 0, H minus2, Opacity 60, Blur 11.88. Hide the white layer and merge visible.

Go back to your preset shapes again and choose a star shape, set the stroke to white and width 3 and the fill to black. Draw out a star, you must be able to fit a letter inside, and also be able to put 4 or 5 in a column next to the legs. Convert to raster layer, Add the same dropshadow as for the tube and make 3 or duplicates. Arrange them to your liking next to the legs. You can rotate them a little with image/rotate/free rotate, 11 degrees to either side.

Take your text tool, stroke nil, fill white, I used Altamonte NF, size 26, create as vector. Use capital letters and place each in a star, convert to rasterlayer.. When you're done, hide the white layer and the one with the legs and merge the stars and letters visible. Apply this inner bevel twice.

When you're happy with your placement, hide the white layer and merge visible. Open the DiamondStrand tube and pick a colour, I chose white. Right click/copy and right click/paste as a new layer onto your square, add a dropshadow. Add a rasterlayer, load your brushes, and pick one or two pretty brushes to go next to the strand. I used white and set the opacity to 100. Again, hide the white layer and merge visible. Move the tag into the center of your canvas. Make the white layer active and add a rasterlayer.

Pick a colour, any colour (lol) from your tube or your favourite one if yours is b/w like mine. I chose a gradient of the shades of purple. Load your brushes and add some to make a background for your square. Change the opacity back to about 75. Make sure you don't go over the edges of your canvas! For the last time, hide the white layer and merge visible. Choose your crop tool and crop layer opaque/ current layer.

Add credits to the artist and yourself. merge all visible. Now it's time for your name. I used the font Incarnation at size 48, stroke white, width 2 and fill black. Create as vector, stretch or squeeze your name until you're happy, add the same inner bevel as before, just once though. Save as a jpg.
