A Scrap Kit, mine is by Jaelop Designs and it's a FTU kit called Sweet Girl.
Font(s) of choice, mine is CK Amelia

You should have some knowledge of working with layers for this.
Open a new canvas, 550x550 and paste Element1 from the kit into it, take your freehand selection tool and go around the frame on the pink circle.
Invert the selection, paste a copy of your tube under the frame, resize if necessary, place the main focus within the selection and hit delete on your keyboard.
If you have transparency showing behind the tube also paste a paper behind the tube and delete the excess. Deselect. Merge visible.
Move this element to a corner. Copy and paste one of the oblong frames, resize it a bit. Select the frame the way you did before and paste your tube mirrored behind there
also. Possibly also paste a paper behind there. Greyscale the tube by going to Adjust/Hue and Saturation/Colorisation and sliding both to 0. Lower the opacity to 50 or thereabout.
Hide the merged layer and merge visible. Unhide merged layer.
Take the wordart from the kit and add to your tag, also add elements to your taste. Take a paper and paste as the bottom layer. Apply a mask, merge group.
Crop merged opaque, apply a dropshadow to all layers, except the masked one.
I resized the entire tag by 85%. Add credits legibly and also your name.
My second example uses two images by ChildressImaging, who is also signed with CDO. The mask is 496 by InsatiableDreams and the font is Spring Light.
